Acelity Promogran® PRISMA Matrix Wound Dressing - 4.34" Sq, 10/bx, 4 bx/cs

Please note: All prices are for cases.

The Solventum Promogran® Prisma™ Matrix (SKU MA028) is a sterile, advanced primary wound dressing designed to address the biochemical imbalances that often stall the healing process in chronic wounds. This specific 4.34 square inch hexagonal matrix is composed of a freeze-dried composite of 55% Collagen, 44% Oxidized Regenerated Cellulose (ORC), and 1% Silver-ORC. Upon contact with wound exudate, the matrix transforms into a soft, conformable, and biodegradable gel that ensures intimate contact with the entire wound bed. This dual-action environment is engineered to bind and inactivate destructive proteases (MMPs and elastase) while the ionically bound silver provides a sustained antimicrobial barrier to protect the wound from infection.

Key Features

  • Advanced Protease Modulation: Specifically targets and neutralizes excess Matrix Metalloproteinases (MMPs) and elastase, which are known to degrade growth factors and stall healing in chronic wounds.
  • Silver-ORC Antimicrobial Barrier: Contains 1% ionically bound silver, providing broad-spectrum protection against common wound pathogens without the cellular toxicity often associated with high-load silver products.
  • Biodegradable Gel Transformation: The matrix naturally resorbs into the wound bed over time, eliminating the need for mechanical removal and protecting delicate new granulation tissue during dressing changes.
  • Physiologically Moist Environment: Maintains an optimal moisture balance conducive to rapid granulation and epithelialization, supporting the natural repair of damaged tissue.
  • Hexagonal Precision Shape: The 4.34 sq in footprint is ideal for mid-sized wounds, providing localized therapy while minimizing product waste in a clinical setting.

Ideal Clinical Applications

  • Diabetic Foot Ulcers (DFUs): Highly effective for stalled diabetic ulcers where high protease levels or bioburden are impeding closure.
  • Venous Leg Ulcers (VLUs): Provides a stabilized healing environment for chronic lower-extremity wounds that require both moisture balance and antimicrobial protection.
  • Pressure Injuries: Indicated for Stage 2, 3, and 4 pressure ulcers that are clear of necrotic tissue but remain in a chronic inflammatory state.
  • Traumatic and Surgical Wounds: Ideal for dehisced surgical sites or traumatic injuries healing by secondary intention that require biochemical re-balancing.
  • Negative Pressure Wound Therapy (NPWT): Compatible with NPWT systems (such as V.A.C. Therapy) to optimize the wound bed microenvironment under the foam and drape.

Important Safety Information

Contraindications: Do not use on patients with a known sensitivity to collagen, silver, or oxidized regenerated cellulose. Not indicated for use on third-degree burns.

MRI Safety Status: Promogran® Prisma is MRI Unsafe. Due to the silver content, the dressing must be removed from the patient before they enter an MRI suite to prevent potential heating or imaging interference.

Wound Preparation: The wound bed must be clear of necrotic tissue and eschar before application. This dressing is a biochemical modulator and is not intended for debridement.

Secondary Dressing Requirement: This is a primary matrix and must be covered with an appropriate secondary dressing (e.g., foam, absorbent pad, or film) to maintain the moist healing environment.

Moistening Protocol: For wounds with minimal exudate, the matrix should be moistened with sterile saline or Ringer's solution prior to application to initiate the gel transformation process.

Change Frequency: Reapply the matrix as directed by clinical protocol (typically every 24 to 72 hours). It is not necessary to remove residual gel during changes; simply cleanse the wound and apply a fresh matrix.
